Discharge Coordinator.Location - East midlands
Daily Rate - Competitive
Start -ASAP
Length - 3 Months
An NHS client, based in the East Midlands, is currently looking for a discharge coordinator, to assess patients leaving a hospital setting, in order to prevent future hospital admission. You will be required to work in close partnership with the ward to find a suitable transfer, from an acute setting into the community.

- Band 5 + Nurse
- Registered General Nurse
- Registered Mental Health Nurse
- Registered Dual Nurse
Key experience and qualifications
- Knowledge of the National Framework.
- Excellent communication and organisational skills.
- Mentoring or practice educator qualification( or preparing to start this)
- Leading, supporting and motivating team members.
