Description
Interim Continuing Healthcare Lead Nurse required for a Essex based Clinical Commissioning Group for a 3 month contract.My Client is seeking to appoint an interim Continuing Healthcare Nurse Assessor for 3 months initially at a competitive daily rate. This position is required to facilitate the undertaking of Continuing Care Assessments, Health Needs Assessments and NHS Funded Nursing Care.
Experience required:
- Experience of working within a CCG
- Registered General / Mental Health Nursing Qualification
- Experience of working in partnership between health and social care
- Experience of developing Care Packages and Specialist Programmes of Care for patients who meet the Eligibility Criteria
- Experience of assessing, planning, implementing and evaluating patient admissions into NHS Continuing Healthcare placements.
