Interim Continuing Healthcare Complex Case Manager required for a London based NHS organisation to start as soon as possible.My client is seeking to appoint a Continuing Healthcare Complex case manager to support the organisation on an interim basis for 3 months initially with the potential to extend beyond.
To be considered for this position, the key skills and experience required are:
- Experience managing complex case loads
- Registered General Nurse / Mental Health Nurse
- Excellent working knowledge of Continuing Healthcare
- Strong knowledge of the Continuing Healthcare framework-
- Experience of working with Multi-Disciplinary teams
This role will involve completing and co-ordinating CHC assessments, involving the client, families and members of the multi-disciplinary team.
