My client is seeking to appoint a Continuing Healthcare Complex Case Manager for Learning Disabilities to support the organisation on an interim basis for 3 months initially with the potential to extend beyond.To be considered for this position, the key skills and experience required are:
- Experience managing complex case loads
- Registered Mental Health Nurse / Learning Disability Nurse / Social Worker
- Strong knowledge of Learning Disabilities
- Strong knowledge of the Continuing Healthcare framework
This role will involve completing and co-ordinating assessments, involving the client, families and members of the multi-disciplinary team. This role will require travel around the local area and on occasion further afield so a valid driving licence will be required.
