My Client, a London based NHS Commissioning Organisation is looking to recruit for a Continuing Healthcare Nurse Assessor to work within Neuro Rehabilitation. This position will be 4 months in length with the view to start as soon as possible.The key responsibilities of this positions are:
- To manage the neuro-rehabilitation cases across multiple boroughs
- Lead on Complex Discharges
- Will play a lead role in the merging of services to create a united team across multiple boroughs
- To manage referrals from a range of sources and determine eligibility for Funded Nursing Care or CHC
- Coordinate and support the assessment process by the MDT and liaise with patients, families, health and social care colleagues and care providers.
