Start: ASAP - Ideally Mid March
Duration: 3 Months (View To Extend)
Rate: £250 - £270 Dependent Upon Experience
My NHS client in the East of England has an exciting opportunity for an experienced CHC Team Lead to join their diverse team for an initial 3 month contract.
Essential Skills:
- Must be a Registered Nurse
- Experience of undertaking care need assessments, care and risk management
- Knowledge and experience working in Continuing Care
- Experience of applying the CHC National Framework
- Experience working with complex patients
- Experience of working in an MDT (Multi-Disciplinary team)
- Must have a valid driving licence.
You will be providing professional leadership to a locality team. You will work closely with and support other members of the Continuing Healthcare Team providing skilled and experienced support to the CHC assessors, Funded Nursing Care Assessors as well as to the CHC Specialist Team Leads, CHC Service Managers, CCG Leads and stakeholders.
The role will also include supporting patients and their families through the CHC process.
Regular travel will be required.
You will be able to manage your own workload and deliver to specific time scales and drive improvement on performance.
