Description
My NHS client based in the South East is currently recruiting for a continuing healthcare practitioner for a 3 month interim project.You will be based mainly in an acute hospital working with the discharge teams and developing relationships with hospital team to ensure length of stays are decreased for Continuing Healthcare clients.
You may be required to:
- Ratify CHC assessments
- Assist with complex discharges
- Work with the CHC procurement teams in identifying placements
- Support the teams in completion of assessments in terms of identifying training needs and reviewing processes
This is an immediate start part time (3 days) role for an initial 3 month period.
