An expecting opportunity has arisen with a Community Healthcare Trust in East Anglia. They have asked me to assist them in sourcing an interim Continuing Healthcare Lead Nurse Assessor (band 7).My customer is seeking to appoint an interim Continuing Healthcare Lead Nurse for 3 months at a competitive daily rate.
This position is required to facilitate the undertaking of Continuing Care Assessments, Health Needs Assessments and NHS Funded Nursing Care.
Experience required:
- Experience with NHS Commissioning Bodies
- Registered General / Mental Health Nursing Qualification
- Experience of working in partnership with health and social care
- Experience of developing Care Packages and Specialist Programmes of Care for patients who meet the Eligibility Criteria
- Experience of assessing, planning, implementing and evaluating patient admissions into NHS Continuing Healthcare placements.
