Description
Interim Continuing Care Learning Disability Nurse Assessor required for a London based CCG for a 3 month contract to begin ASAPMy client, a Greater London based Clinical Commissioning Group are seeking to appoint a Learning & Disability Nurse Assessor to help bridge the gap whilst they recruit to the position permanently.
To be considered for this position you must have the following skills and experience:
- Registered LD Nurse
- Experience of carrying out assessment of needs using decision supporting tools and making recommendations to CCG panels
- Case manage complex health care packages for CHC learning disability clients
- Experience of liaising with multi-disciplinary teams
- Working to the guidance of the NHS framework
