Description
Interim Continuing Care Nurse Assessor required for a 3 - 6 month period with a large Clinical Commissioning Group in the South West of England.My client, a large Clinical Commissioning Group based in the South West of England are looking to recruit for a Nurse Assessor to join the team on an interim basis whilst they recruit substantively to the position.
The post holder will would be required to -
- Coordinate the assessment process
- Facilitate the multi-disciplinary team meetings
- Ensure appropriate packages of care are put in place.
The candidate would need to have excellent knowledge of the Continuing Healthcare agenda and processes and ideally an understanding of the Transforming Care agenda.
