Swim Social Work Recruitment are looking to recruit a Social Worker/Care Manager to join an Older Peoples Team based in Greater London.You must hold GSCC registration and have substantial post qualification experience. The client will not accept CV's if you do not hold this registration/experience, so please apply if applicable.
Your job will be;
*To work with a caseload of vulnerable people over 65
*To assess needs based on the directorate's eligibility criteria
*To set up care packages including - direct payments, short or long term care, day care service, respites and home care
*To undertake Mental Capacity Assessments
*To monitor and reviewing the quality of care packages
*To work in partnership with other professionals from the NHS, Housing, Police and other Public Services to ensure services users' care, safety and wellbeing
