Swim Social Work Recruitment are looking to recruit a Social Worker/Care Manager to join an Adults and Older People Community based team in Bexley. This is a full time post for 3 months initially.You must hold GSCC registration and have at least 2-3 years post qualification experience. The client will not accept CV's if you do not hold this registration/experience, so please apply if applicable.
*Must be a car driver*
Your job will be;
*Undertaking assessments , including adult safeguarding, hospital discharge, individual budgets, long term care arrangements etc
*Preparing direct reports for panel with reference to eligibility for continuing health care funding
*Maintaining accurate and up to date cae recordings
*Providing care management services to clients over the age of 18 with physical disabilities and those over 60/65 with physical and/or mental health difficulties
*Managing a caseload of vulnerable adults and older people
*Monitoring and review the quality of care packages
*Working in partnership with other professionals from the NHS, Housing, Police and other Public Services
