Swim Social Work Team are looking to recruit a Qualified Social Worker in the Children with Disabilities Team based in Barnet. This is a 3 month, full time position.

Your job will be to:
*Understand and be able to respond sensitively to the particular vulnerabilities of children with additional needs, to act as an advocate for their needs when appropriate to do so. Identify and facilitate appropriate services and resources.
*Regularly monitor and review services to ensure they are provided effectively and in accordance with need.
*Provide services, advice and continuing support to children and young people, their families/carers according to an agreed service plan; taking into account the requirements of the Disability Discrimination Act 2005.
*Undertake carers assessments in line with the Carers (Equal Opportunities) Act 2004.
*Participate in case conferences and child in need planning meetings.
*Work with Adult Services to ensure effective transition.
*Work with partner agencies to provide packages of support to enable children and young people to remain at home where possible.

You must have the following experience:
*At least 3 years post qualification experience in social work
*Minimum of 6 months in a Children's Disabilities Team

You will need the following qualifications:
*Social Work degree / DIPSW
*Full GSCC registration
