Description
Swim Social Work Team are looking to recruit a Community Care Worker in the Mental Health Team based in Leicester. This is a 6 week, full time position.Your job will be to:
*Receive initial contacts and referral for adults social care
*Screen initial contacts, make preliminary identification of need and match assessment response to this.
*Undertake work with service users and their carers within the context of the Specialist Team
*Promote, within agreed priorities, the independence of disabled/older people (including children) and to offer support to carers
*Undertake assessments and arrange services to meet identified need in line with the Department's eligibility criteria
*Monitor and review cases as appropriate
You must have the following experience:
*Previous experience of working in Mental Health Social Work team
*Carrying out assessments, monitoring and reviewing cases
*Experience of maintaining effective and confidential records
*Experience of working in multi-cultural communities
*Ability to work creatively and flexibly to formulate appropriate care packages
