Swim Social Work Team are looking to recruit a Qualified Social Worker in the Mental Health Team based in Ealing. This is a 3 month, full time position.

Your job will be to:
assess and review service users and carers within the framework of the Care Programme Approach
develop care plans to meet the assessed social care needs, using resources available in the statutory and voluntary sectors
provide therapeutic relationships to enable clients to achieve independence, maximise potential, and resolve difficulties within their physical, emotional and social environment

You must have the following experience:
at least 3 years post qualification experience with 1 years experience of working in a mental health team

You will need the following qualifications:
social work degree / DIPSW
GSCC registration
