Swim Social Work Recruitment are looking to recruit a Care Manager to join an Adults with Learning Disabilities Team based in Lambeth. This is a full time, temporary post for 2 months.

You must hold GSCC registration and have substantial post qualification experience. The client will not accept CV's if you do not hold this registration/experience, so please apply if applicable.

The role is busy and demanding therefore you will be required to be proactive, a competent communicator both orally and in writing. You will supported and guided by the Duty Practitioner Manager. Work will be prioritized in consultation with the Duty Manager.

Your job will be to;
*Receive new referrals; mainly safeguarding referrals
*Hold a caseload
*Attend SG meetings
*Undertake FACS assessments
*Write reports/assessments
*Respond to enquiries
*Record accurate data/notes on the Framework
