Swim Social Work Recruitment are looking to recruit a Social Worker to join a Physical Disabilities Team based in Camden. This is a full time post for 3 months initially.You must hold GSCC registration and have at least 2-3 years post qualification experience. The client will not accept CV's if you do not hold this registration/experience, so please apply if applicable.
This post is with an integrated team so a knowledge of MDTs would be helpful. The role will mainly be office based but will involve visits to users in their homes or social care settings and visits to partner agencies.
Your job will be to;
*Manage a case load of up to 25 customers living with complex needs physical disabilities and cognitive deficits.
*Participate in the duty system (1 day/month)
*Undertake assessments of service users (adults over 17 who have neurological conditions), including needs assessments
*Provide a comprehensive, outcome-focused care management service
*Deliver exceptional social care services to adult service users and their carers within a defined budgets, enabling them to make their own decisions
*Ensure equality of access to services
*Provide an excellent assessment, care planning, care management and Social Work service in either a hospital or community setting
*Manage risk whilst promoting time limited care by application and understanding of relevant legislation and best practice guidance
*Be imaginative in commissioning of services, in the first instance seeking alternative solutions to Council funded care
*Ensure accurate inputting of management information
You will have;
*A knowledge of relevant Social Care and Health legislation policies and procedures
*Experience as a specialist or generalist Social Worker
*A knowledge of FrameWork I
