Swim Social Work Recruitment are looking to recruit a Social Worker to join a Mental Health Team based in East Sussex. This is a full time role for 2 months initially.You must hold GSCC registration and substantial post qualification experience. The client will not accept CV's if you do not hold this registration/experience, so please apply if applicable.
Your job will be;
*To assess, interview and use care planning skills
*To organise and prioritise a complex caseload
*To utilize your social work skills effectivly, especially with those with memory loss and dementia and their carers
*To liaison and plan with colleagues and other agencies
*To construct and enable effective care plans
*Ability to work both independently and as part of a team
*To be aware of issues in working with adults with complex needs, e.g. ageing, physical disability, mental ill health and dementia
*To be awre of the NHS and Community Care Act, Mental Capacity Act.
*To work in line with social work practice and procedures for people with dementia and their carers
*To undertake assessments of need
*To review and monitor client needs and service delivery
*To work constructively with others to ensure wellbeing for those with a diagnosis of dementia
