I’m trying to create a search form that will pull data from MYSQL database and Display results on my WP theme I will be very happy to communicate with anyone who is willing to help me out. If you know what you are doing it should take you less than 2(two) hours. I have found multiple codes and tutorials online but I can’t execute them. I know little bit about WP platform but I’m no expert in MYSQL database. Here is a simple BLANK form that I created that I want to display:

Select a State

Select a County

Select a City

Select a Company

Agents Type

The form function will be like this:
IF select a state, the counties within that state will display THEN the cities of the county selected will display and so fourth. Once you click submit QUERY the result will display in a separate page. For instance, if you select Philadelphia as city, the result should display: http://www.website.com/philadelphia/

I don’t have big budget but rather limited, let me know the cost and I will be more than happy to talk.
