Dynamic Search Form Using Chained Select: PHP/MYSQL and AJAX
Hello,
I have been trying to build a Chained Dynamic Search Form using php/mysql and Ajax but failed. I am somewhat familiar with WP, I just lack execution. Here is the issue:
I want the form to display the followings in order:

Select a State

Select a County

Select a City

Select a Zip Code

What I want to accomplish is populate fields Dynamically. For instance, If someone Select ALASKA as a state, The next field should GET results for all the Counties in ALASKA, and once you select the desired County, the next fields should DISPLAY all CITIES in that specific County and the same goes for Zip Codes. When a person clicks: QUERY (Or Submit) Results should display in a friendly manner: URL friendly like this one: http://www.website.com/State/County/City/Zip/

I would really appreciate any help here. The pay is $50 for this Job. Please DO NOT contact me if your are hoping to charge me more, if you know what you are doing it shouldn’t be hard to explain to me how to proceed, I can do it myself I just need instructions.
