My client, a top operator here in Houston, is urgently looking for a BOP RCFA lead. Please see the description below.

The following skills/experience is desired:
  • 4-year engineering or technical degree
  • 10+ years experience with BOP/Riser and pressure control systems
  • Familiarity with tool/equipment design, quality/reliability and maintenance and/or investigation
  • Failure analysis experience
  • Training in formal Root Cause Analysis processes
  • Strong communication skills (both verbal and written)


The following general job description applies:
  • Conduct multiple equipment failure investigations at a time according to the defined Root Cause Failure Analysis (RCFA) process.
  • Witness and represent Shell concerns/interests during incoming testing, disassembly and inspection activities following equipment failures. This may include coordinating participation and defining responsibilities for qualified third party representatives to witness incoming testing, disassembly and inspection activities following equipment failures when RCFA/TAT members cannot be present.
  • Deliver timely preliminary failure analysis reports to the appropriate well delivery personnel. These reports provide the initial findings, interim recommendations and the forward plan.
  • Work closely with RCFA Team members, Subject Matter Experts and the vendor's failure response focal points during RCFA's to develop the terms of reference, focus investigation tasks, evaluate data and results, and develop conclusions, corrective actions and recommendations.
  • Facilitate independent testing and analysis during investigations, as necessary. The vendor will often be capable of arranging for the required testing and analysis, but this work must be evaluated and approved by the RCFA/TAT Analyst.
  • Prepare RCFA reports in a timely manner via the prescribed format. Upload approved reports into Shell's reports database and link the reports to the RCFA/TAT's website.
  • Inform RCFA/TAT Leader of key issues and improvements identified through RCFA.
  • Prepare and deliver RCFA highlights presentations for engineering network meetings.
  • Prepare notices/cautions and key investigation updates for the quarterly RCFA Awareness Bulletins, which are directed at well delivery teams to mitigate risk of repeat failures.
  • Participate in meetings with well delivery personnel to help summarize RCFA findings, corrective actions and recommendations. Such meetings may include well planning sessions, after action reviews and drilling engineering network meetings.
