Description

Here is the “story” if you will:
  • convert the enclosed template to a web form with “must-fill” blanks.
  • place the webform on a new page at www.networkscre.com so that it can be manipulated in the future using the existing WordPress software.
  • The “Submit” button on the form should create an image file (PDF or JPG) that is automatically stored in either a dropbox, Drive account or emailed to me automatically.
  • The form should clear after the submit button for the next entry.
