Description
Position Summary:
The health educator is a part time, exempt employee expected to work 24-28 hours per week including some weekends and evenings; required to work in the office and travel to various locations in the community.
Duties and Responsibilities:
- Teach Diabetes Self-Management Education classes and workshops

Provide 1:1 individual Diabetes Self-management education to clients and their support team.

Provide diabetes prevention and awareness presentations throughout the community

Facilitate Empowerment Groups

Develop and revise diabetes/nutrition-related education materials
-
Conduct diabetes risk assessments and screenings at community health fairs and in the office

Facilitate use of plain language and health literacy concepts in client education materials/programs

Answer phone questions

Assist walk-in clients

Assist clients to access blood glucose testing supply program

Conduct Chronic Disease Self-management session.

Represent Diabetes Partnership of Cleveland at meetings and on coalitions/committees

Develop and manage diverse diabetes education programs and materials including:

Diabetes Ambassador Program: train, coordinate, engage, and deploy health care professionals providing community blood glucose screening and education and community members to support health education, outreach and fund raising activities in the community.

Coordinate the Summer Internship Program.

Manage health literacy initiatives including material and class development.

Provide or support other programming as directed by the Program Director.

Minimum Required Qualifications:
Bachelor's Degree in Nursing or Nutrition.
Maintain current licensure and registration.
Able/willing to work in office conditions and diverse areas throughout the community
Basic competency in Microsoft Word, Excel, and Outlook
Able to lift up to 30 pounds
Ability to work effectively with the team
Ability to manage multiple projects
Ability to meet deadlines
Exhibits punctuality in attendance
Ability to maintain confidentiality
Attends to safety in performance of duties
Maintains high degree of accuracy in work and reporting

Preferred Competencies:
Experience working with type 1, type 2 and/or gestational diabetes
Prior extensive knowledge about diabetes
