There is an exciting opportunity within Continuing Healthcare for case manager/triage nurse to join a CCG located in East Midlands. My client is looking for a Registered Nurse with experience within Continuing Healthcare.
In this role you will be required to create the packages of care and also to review cases that were previously not completed. You will be actively using the DST framework and the checklist to review and create the packages of care. Experience within complex cases is highly desired.
