Description
My NHS client based in London is currently recruiting for a senior continuing heatlhcare nurse assessor to join their fast paced team.- The post holder will work as part of the joint team, and with patients and families, to ensure that the healthcare needs of patients referred for continuing health care (CHC) eligibility are identified and met in accordance with the National framework for NHS continuing healthcare and NHS funded nursing care
- Provide clinical expertise, advice and guidance to all CHC cases
- Clinical responsibility for all CHC cases
- Lead care and support planning when required ensuring the optimal use of NHS resources
- Lead care and support planning for Personal Health Budgets
- Present CHC cases at panel ensuring sufficient and robust evidence supports assessment recommendations and provide a thorough understanding of the full caseload
- Provide expert CHC advice and support to the safeguarding team and attend safeguarding meetings when required
- Undertake or support urgent reviews associated with provision of care for patients in care home settings or care provided at home, as required
- Responsible for the management, training and objective setting of the Continuing Healthcare Nurse
This is an immediate start 3 month role. Daily rate is negotiable.
