Role: CHC Nurse AssessorLocation: East of England
Start: ASAP - Ideally Mid March
Duration: 3 Months (View To Extend)
Rate: £250 - £270 Dependent Upon Experience
My NHS client in the East of England has an exciting opportunity for an experienced CHC Nurse Assessor to join their diverse team for an initial 3 month contract.
Essential Skills:
- Must be a Registered Nurse
- Knowledge and experience working in Continuing Care
- Experience of applying the CHC National Framework
- Experience working with complex patients
- Experience of working in an MDT (Multi-Disciplinary team)
- Must have a valid driving licence.
You will be working with patients and their families who are often at a vulnerable stage of their lives. This will also ensure the individual's needs are assessed and met in accordance with the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care
You will be able to manage your own workload and deliver to specific time scales.
Regular travel will be required.
