Description
Reporting to the CHC Team Leader, the successful candidate will be responsible for implementing National Policy and the framework for NHS Continuing Healthcare (CHC). The postholder will lead on the assessment, procurement and review of patients presenting through the fast-track pathway facilitating and co-ordinating end of life care patients throughout the hospital discharge pathway and working collaboratively with key partners to achieve the best outcomes for patients.In addition to this, you will be competent in the assessment and review of patients across the CHC pathway using the National Framework Guidance and tools, ensure that needs have been adequately assessed and progressed through the appropriate care pathway, and to ensure that all reviews, and reassessments include appropriate reports and evidence to support the level of care identified, including a review of the placement/package of care and to ensure that all care needs are being met.
You will also provide expert case management to support both Health and Social Care staff who are involved with the patients care provision and planning.
The successful candidate will be an experienced Service or Nurse Manager with a strong grasp of CHC, ideally a Nursing or Medical background and be a strong communicator with the ability to influence at all levels.
