Registered Learning Disabilities NurseCHC Nurse Assessor within NHS
Location - North West
Start - ASAP
Contract Length - 3 months
An NHS Client is looking for CHC Nurse Assessors to carry out FNC/CHC Assessments in the local area. This is a great opportunity to get more hands on experience within Continuing Health Care. Ideally you will be a Registered Learning Disabilities Nurse but we are also looking for General and Mental Health Nurses
You will:
- Be Carrying out assessments to CHC/FNC Processes
- Attend Multi- Disciplinary meetings to contribute to the DST as part of CHC National framework
- Be used to managing own caseload and working independently
- Have good Knowledge Continuing Care
- Experince of working within a MDT
- Knowledge of CHC National Framework
- Be able to travel across locality
- Have a Registered Learning Disabilities Nurse NMNC Pin
- Have experience working with complex cases within Learning Disabilities
- Have access to own transport
